The Tribal Health Transformation Partnership is led by the Cayuse Holdings family of companies, 100% owned by the Confederated Tribes of the Umatilla Indian Reservation.

The Partnership supports Tribes in transforming healthcare through culturally-grounded strategies that strengthen workforce systems, care delivery, and community capacity. Work 100% funded via the Rural Health Transformation Program.

Tribes are being invited into unprecedented levels of health system planning and investment. States are dedicating substantial funding for Tribal-led initiatives, workforce development, digital health, and culturally-aligned care delivery models.

 

With opportunity comes pressure. Tribes are being asked to absorb additional responsibilities all while continuing to deliver care in under-resourced environments.  

The risk is not lack of vision. The risk is capacity strain. Tribes should not be forced to choose between accessing transformational  funding  and protecting long-term stability and sovereignty.

Phase 1: Awareness & Program Design

We help you understand the federal Rural Health Transformation Program - what your state will fund and when - then help you create an application plan that is responsive and culturally grounded for your community.

Phase 2:
Apply

We help you write a winning grant application by providing feedback and insight on how to align narratives, budgets, metrics.

 

With around $200M available per state in 2026, we help ensure your organization is positioned for successful funding acquisition.

Phase 3: Implement

If you’re awarded a grant, we can act as your subcontractor to implement your program(s).

Strategic clinical and infrastructure transformation. Implementation fees are fully covered by the RHTP grant, allowing Tribal health leadership to deliver measurable health improvements without financial burden.

Phase 4: Sustain

We work with a mindset to invoke long-term transformation and sustainability of your tribal health programs.

We help you secure long-term generational health equity. We establish robust monitoring systems to ensure your transformed Tribal health programs remain sustainable throughout the 5-year federal program and beyond.

Zero Cost Entry

Awareness, program design, and grant application support are provided at zero cost. Additionally, implementation fees are covered by the RHTP grant to ensure long-term sustainability.

Significant Funding

States are estimated to receive around $200 million each in 2026. This $50 billion federal pool is one of the largest rural health investments in history, open in 2026 for Tribal applications.

A $50 Billion
Federal Investment

The Rural Health Transformation Program (RHTP) represents a historic opportunity for Tribes to enhance healthcare for their communities. Learn more about the federal Rural Health Transformation Program (CMS RHTP overview).

The RHTP represents one of the largest federal investments in rural healthcare in U.S. history—$50 billion between 2026 and 2031. This initiative goes beyond temporary fixes to build sustainable, community-rooted systems that expand access, stabilize hospitals and clinics, strengthen workforce pipelines, and improve chronic disease and specialty care for rural populations. 

 

For Tribal Nations, this moment carries profound significance. Many of the most rural, highest-need communities are Tribal or Tribal-adjacent, yet have been systematically under-resourced for decades. States, including Alaska, Oregon, Washington, and Idaho, have earmarked over $100M of funding specifically for Tribal Health providers. RFPs for funding programs in all states are expected to emerge in Q1 and Q2 of 2026.
